Validation of Race-and Recurrence Specific Serum Biomarkers for Breast Cancer

African American (AA) women have an approximately 40% higher risk of breast cancer (BC) mortality postdiagnosis, are often diagnosed with more aggressive breast cancers and have worse survival outcomes than their Caucasian American (CA) counterparts. Despite surgery, 10% of BC recur and 18% of recurrent BC occur in the AA group. The biological determinants of BC racial disparity are unclear and there are no specific biological markers and no reliable methods to predict recurrence on an individual basis. Therefore, our strategy is to develop and validate a non-invasive multi-marker panel that can serve as a “liquid mammogram” that may especially benefit patients with race-specific interventions aimed at reducing the risk of metastasis and recurrence in Breast Cancers (BrCa). Using serum from 240 BrCa patient cohort from Walter Reed Military Hospital on antibody and reverse phase protein microarray, we have identified in the case of AA vs CA patients, the AA-specific proteomic bio-signature as VEGFR2, Rb, MEK2, JunD, SGK and PTEN. Using 21 recurrent patients from the above cohort, we have identified several key epigenetic (HDAC5, HDAC7, HDAC9), inflammatory (serum amyloid A (SAA), and IL-18) and NF-κB in the serum of breast cancer recurrent patients compared to non-recurrent patient. In addition, proteogenomic data identified vitronectin, EXOSC4, TGFBR2 and IL-8 as race and recurrent specific biomarkers activating the PI3K/AKT pathway. These candidate biomarkers that discriminate between recurrent and non-recurrent patients have a ROC value between 0.71 and 0.86. Our study provides the foundation to develop a robust, cost-effective, quantitative tool for the ultrasensitive, multiplex detection of race specific breast cancer recurrence markers for personalized medical care in a clinical setting. Over the long-term, this work may improve patient stratification for treatment in BrCA patients.
